How do I add a new SSDR Offense Type Code?

Go to Discipline System > Maintenance – Discipline > Incidents.

Scroll to the bottom of the Incident Code table to the first empty field and enter the new code.

  • Enter the Incident code. The Incident Code can be the same as the State Code but does not have to be.
  • Enter the Description for the code to match the one provided by the DESE.
  • Enter the State Code that should be reported in the SSDR report for this Incident.
  • Select the Incident Type that is appropriate for the Incident.*(See note below)
  • Click Submit to save the new code and update the table.

* NOTE: The Incident Type must be set to Report to DOE = Yes in order for the Incident Code to be included in the SIF transmission or Legacy flat file export for SSDR.

In the example below, both the DOE and School Incident Type Codes are set to Report to DOE = Yes because all “school related” incidents (ie. Lying) have the potential of being reported to the DESE if the incident resulted in a suspension, removal, or expulsion.
